Beyond Horizons works closely with referrers to assess each young person’s individual needs and determine whether we can provide a safe and appropriate placement.
Our team reviews the young person's needs, risks, strengths, cultural considerations and placement requirements.
Where BHYFS can safely meet the young person's needs, an appropriate placement is identified based on compatibility, staffing requirements and available capacity.
The care team collaborates on initial goals, risks, routines, health, education, cultural considerations, family contact and other relevant supports.
Our practitioners are briefed and provided with the information and preparation required to safely support the young person.
Where circumstances allow, the young person can meet relevant staff and become familiar with their new environment before transitioning.
Meet and greets may not be possible for emergency placements.
Following placement, we maintain communication with the young person and their broader care team and review supports as the placement develops.
