Beyond Horizons Youth & Family Services provides safe, stable and nurturing out-of-home care for children and young people who are unable to live safely within their family home.
We understand that every young person entering care has their own experiences, strengths, relationships, culture and support needs. Our approach focuses on providing consistent day-to-day care while working collaboratively with Child Protection, families and the broader care team to support each young person’s safety, stability and development.
Health & Wellbeing: We support each young person’s physical and emotional wellbeing, including attending health appointments, medication support where required, healthy routines, nutrition, sleep, personal care and access to appropriate health professionals.
Emotional & Behavioural Development: Our team provides trauma-informed and relationship-based support to help young people feel safe, build trusted relationships, understand their emotions and develop positive ways of managing challenges.
Education & Learning: We support school attendance, educational engagement, homework, communication with schools and access to alternative education, training or employment pathways where appropriate.
Family & Social Relationships: Where safe and consistent with the young person’s care plan, we support connection with parents, siblings, family, kin and other significant people. We also encourage healthy friendships, social participation and positive relationships.
Identity, Culture & Belonging: We respect each young person’s culture, identity, language, religion, sexuality, family background and individual experiences. Aboriginal children and young people are supported to maintain and strengthen connections to Country, culture, family, kin and community in accordance with their cultural plan.
Social & Community Participation: Young people are encouraged to participate in sport, recreation, hobbies, cultural activities and community experiences that reflect their interests, build confidence and promote a sense of belonging.
Daily Living & Self-Care Skills: We support young people to develop age-appropriate skills such as personal care, cooking, cleaning, shopping, budgeting, transport, decision-making and managing everyday responsibilities.
Individual Care & Placement Planning: Every placement is approached individually. BHYFS works with the young person and their care team to understand their needs, risks, strengths and goals and to implement their agreed Care and Placement Plan.
Support is reviewed as the young person’s needs change, with their voice and preferences encouraged throughout the care process.
Supporting Culture & Connection: For Aboriginal children and young people, culture is an important part of care. We support the implementation of individual cultural plans and work respectfully with family, kin, Aboriginal organisations and relevant cultural supports to strengthen connection to Country, culture and community.
Preparing for the Future: For older young people, our care also focuses on preparing for adulthood. This may include developing independent living skills, financial literacy, education and employment pathways, healthy relationships, community connections and planning for future accommodation.
Our Goal: Our goal is to provide more than a placement. We aim to create a safe and supportive home where each young person feels heard, respected, connected and valued, while giving them the care, stability and opportunities they need to work towards positive long-term outcomes.
